PRIMARY FUNCTION

Responsible for conducting client assessments, developing and implementing care plans, and providing referral services to older adults at risk of institutionalization. Responsible for providing advocacy and linkage of clients to maintain self-sufficiency. Position may from time to time be stressful and require a high demand of performance.  May perform other duties as assigned.

 

ESSENTIAL FUNCTIONS

  • Conducts in home assessments to determine and verify eligibility for individuals under the Title III Nutrition program
  • Provides referrals for other services to eligible and ineligible clients
  • Coordinates the development of the Individual Service Plan for all eligible persons and documents the plan
  • Provides case management services for assigned cases
  • Documents and completes case management reports and paperwork required by contracts and state laws
  • Develops and reviews solutions for participant cases and effective program strategies with other team members
  • Establishes and maintains both paper and electronic case records
  • Completes required client assessments and redeterminations in a timely manner
  • Promotes advocacy services
  • Liaison between program and other agency programs, community agencies and services
  • Responsible for the coordination of services and development of referral relationships in order to enhance resources
  • Provide accurate documentation of services, including identifying of resources, progress notes and summary of services
  • Substitutes for home delivered meals driver adhering to all established procedures for this position
  • May drive agency or personal vehicle on company business
  • May perform other duties as assigned

MINIMUM QUALIFICATIONS

Suitable work experience may be considered as transferable skills in order to meet minimum requirements of the position and will be considered by the Corporate Director of Human Resources.

  • Bachelor's in human services
  • 1 year of experience in related field
  • Bilingual English/Spanish, verbal and written
  • Proven record of working effectively with families of varied lifestyles, backgrounds and ethnicities
  • Proven skills utilizing Microsoft Office Suite
  • Valid Arizona Driver License, proof of insurance, Arizona Motor Vehicle Division 39 month Motor Vehicle Report, and reliable, safe transportation
  • Obtain State of Arizona Level One Fingerprint Clearance Card
  • Ability to obtain CPR, First Aid certification
  • Proven strong communication skills oral and written are clear, concise, and in an organized fashion using appropriate style, grammar and tone
  • Proven ability to work in neighborhood settings with limited supervision

 

DESIRED QUALIFICATIONS

  • Master's in Human Services or related field
  • Additional years of directly related experience
