Position Purpose

Provides direct services including counseling, case management and advocacy for Deaf, Hard of Hearing and Deaf-Blind populations in the areas of vocation, life management, personal growth, and mental health concerns in community settings, home residences and/or at job sites.  Administers individual, family and group counseling, coordinates and facilitates community training and education classes, and maintains necessary case records and client file documentation.  Major emphasis is placed on providing services to Deaf-Blind individuals and coordinating, training, and scheduling Support Service Providers. Position may from time to time be stressful and require a high demand of performance.  May perform other duties as assigned.

 

Essential Functions

  • Completes mental health evaluation/assessment on Deaf, Hard of Hearing and Deaf-Blind adults, youth and children, with supervisor's oversight
  • Provides intake services to include reviewing referral information, completing Case History (biopsychosocial), Intake, Confidentiality, Consent to Treat, Authorization to Release Information, and other forms, presenting information to team at weekly intake meetings, and completing and implementing consumer service treatment plans
  • Provides direct service to individuals in community settings, home residences, and/or at job sites
  • Provides individual, family and group counseling (substance abuse/dependence, women's groups and parent groups)
  • Provides support and assistance, training, and case management to Deaf-Blind consumers in the areas of life management, independent living skills, how to use a Support Service Providers effectively, and vocational options
  • Coordinates, trains and supervises Support Service Providers
  • Coordinates and facilitates various processes and community education classes
  • Provides advocacy in the areas of public benefits programs, education and working with employers and other systems in the community
  • Maintains case records and documentation for each consumer
  • Participates in training and in-services to enhance professional growth
  • Attends monthly Deaf-Blind meetings and statewide task force Deaf-Blind meetings
  • Implements, reviews and maintains CARF procedures and adheres to DRS and CCS policies and procedures
  • May drive agency or personal vehicle on company business

 

Minimum Qualifications

Suitable work experience may be considered as transferable skills in order to meet minimum requirements of the position and will be considered by the Corporate Director of Human Resources.

  • Bachelor's in Counseling, Rehabilitation, Social Work or related field
  • Fluency in American Sign Language and various manual communication modes
  • 2 years of experience working with Deaf, Hard of Hearing and/or Deaf-Blind persons in a professional counseling and/or case management capacity
  • Obtain State of Arizona Level One Fingerprint Clearance Card and pass drug screening
  • Valid Arizona Driver License, proof of insurance and Arizona Motor Vehicle Division 39- month Motor Vehicle Report

 

Desired Qualifications

  • Master's in Counseling, Rehabilitation, Social Work or related field
  • Additional years of directly related experience
  • Understanding of various equipment and technology available to Deaf-Blind individuals
  • Specialized training in working with Deaf-Blind individuals
  • Experience designing and facilitating a technology plan for Deaf-Blind individuals
  • LPC, LISW, LAC or NBCC
