 Responsible for scheduling and transporting clients to and from health care appointments.  Orders medical supplies and maintain inventories.  Works with licensed health care professionals and clients; coordinates medication oversight and configuration on Medication records.  Attends ISP meetings, reports on medical and medication status of all individuals served.  Position may from time to time be stressful and require a high demand of performance. May perform other duties as assigned.


Client Support

  • Schedules and transports clients to and from health care visits
  • Communicates results of health care visits to team members
  • Documents appointments in the Therap documentation system
  • Ensures follow up recommendations are implemented
  • Works with licensed health care professionals, clients and or guardians; coordinates the   medication requirements of residential clients, including set up of medisets and preparation of medication records monthly, to include the monthly configuration of the Medication records in Therap
  • Assists with the preparation and presentation of Individual Support Plans (ISP) and Medication Reviews
  • Complies with all DDD monitoring documentation requirements, progress, and outcomes of clients regarding medical conditions, evaluations and follow up. 
  • Orders required adaptive equipment
  • Works directly with insurance companies re: client eligibility, coverage and approvals for services
  • Implements and follows Individual Support Plan (ISP)
  • May drive agency or personal vehicle on company business


  • Communicates with health care professionals, guardians, and program staff to ensure all licensing documentation requirements are completed according to policy
  • Provides job related training to staff, as directed
  • Complies with all DD Monitoring documentation requirements
  • Assists with maintaining accurate and complete client files, progress reports, and medication reports required by policy and Residential monitoring
  • Checks Medication records at least weekly for completion and accuracy. Follows up with  staff to insure appropriate documentation of MARs 
  • Assists with site compliance with DES, DDD, and DHS licensing regulations, as well as     all Corporate, Agency, and Program policies and procedures
  • Attends all required resident, staff, and in-service training and/or meetings

Residential Site Operation

  • Ensures resident adaptive equipment and Durable Medical Equipment (DME) is properly maintained and used by staff.  Provides training to staff is appropriate use
  • Maintains living environment in a clean, sanitary, and safe condition, and completes home safety checklist, as directed
  • Provides information and training to staff and clients regarding medical issues and needs, prevention of disease and first aid

 Suitable work experience may be considered as transferable skills in order to meet minimum requirements of the position and will be considered by the Corporate Director of Human Resources.

  • High School
  • 1 year of experience in an independent living setting or residential or day program working with persons with disabilities in a Lead position
  • Proven ability to lift 60 lbs 20% of the time
  • Minimum of 18 years of age as required by licensing
  • Obtain State of Arizona Level One Fingerprint Clearance Card and pass drug screening
  • Valid Arizona Driver License, proof of insurance and Arizona Motor Vehicle Division 39-month Motor Vehicle Report

  • Additional years of direct related
  • Certified Nursing Assistant or other related health care licensing
