Position Purpose

Responsible for the collection and input of all data elements for the program. Responsible for creating required reports for AzPac, DES or other sources. Position may from time to time be stressful and require a high demand of performance. May perform other duties as assigned.


Essential Functions


  • Enters all referral forms; demographics information; substance abuse assessments; progress reports; UA results; closures; and any other assigned information to the web portal as assigned or scheduled
  • Enters all monthly progress reports, referrals, assessments, service plans, and any other assigned documents into CHILDS.
  • Creates or provides data for all necessary reports to AzPaC, DES, or other sources
  • Follows up with direct service providers to ensure that all data are received in a timely manner
  • Creates missing data reports at least monthly and communicates with staff regarding the missing information. Communicate with program management staff about missing information and monitor any trends
  • Understands and follows internal and contract guidelines for data collection, entry, and reporting.
  • Attends monthly State Provider Meetings as scheduled or required
  • Participates in any necessary training, both as required by the company and by the contract
  • May drive company or personal vehicle on agency business


 Minimum Qualifications


Suitable work experience may be considered as transferable skills in order to meet minimum requirements of the position and will be considered by the Corporate Director of Human Resources.


  • HS Diploma or GED
  • One-year experience in data entry or human services
  • Must be able to work as part of a diverse group.
  • Must have accurate computer and data entry skills
  • Proven skills utilizing Microsoft Office Suite
  • Possess the ability to relate well to families of varied lifestyles, backgrounds, and ethnicities
  • Experience with multi-problem families
  • Ability to work in a fast-paced environment with moderate interruptions
  • Ability to stoop, bend, squat, sit on floor and ambulate over uneven floor surfaces including stairs and steps
  • Must be able to visually and auditory assess and interact with clients to ensure their safety and well being
  • Obtain State of Arizona Level One Fingerprint Clearance Card
  • Valid driver's license for State of Arizona, proof of insurance and minimum standard 39 month MVR report
  • Ability to lift up to 25 pounds
  • Travel within the state may be required


Desired Qualifications


  • AA Degree in related field
  • Additional years of directly related experience
  • Bilingual, English / Spanish verbal and written
