PRIMARY FUNCTION

Coordinate residential clients with care, supervision, support, and daily living skills for individuals who have multiple disabilities. Responsible for scheduling and transporting clients to and from health care appointments, works with licensed health care professionals and clients; Coordinates medication requirements of residential clients. In the absence of the Site Program Manager, oversees daily program site operations. Responsibilities will include overseeing all daily routines of residence are followed and that all resident/staff coverage occurs at the residential program site. Position may from time to time be stressful and require a high demand of performance. May perform other duties as assigned.

ESSENTIAL FUNCTIONS

Client Support

Schedules and transports clients to and from heath care visits

Documents all medical/therapy appointments for each resident served on the THERAP electronic calendar.

Ensures follow up medical and or therapy recommendations are implemented

Works with licensed health care professionals regarding individual client needs. Communicates with guardians, team members, as appropriate.

Provides information and updates to all staff working with resident regarding changes/updates to client medical care needs, interventions, medications etc.

Documents all results of medical appointments using the CLP Medical appointment form. Insures Physician orders are appropriately communicated.

Coordinates the medication requirements of residential clients, including set up of med-isets weekly and preparation of medication records monthly using the Therap MARS documentation system.

Tracks medication inventory at the site and orders to insure medication is always available at the site

Picks up medications for all residents or assigns to a designated staff member.

Prepares monthly medication records for electronic documentation and signature of all staff providing medication assistance to residents

Changes the Medication Appointment Records (MARS) as medications are changed, discontinued or dosage adjusted.

Assists with the preparation and presentation of Individual Support Plans (ISP) and Medication Reviews

Provides written summaries of all medical and therapy appointments to team members during routine ISP meetings

Assists with the planning for age appropriate group and individual outings

Complies with all licensing documentation requirements, progress, and outcomes of clients

Assists residents on a daily basis with personal care such as hygiene or first aide, either as directed by client, care plan, or Individual Support Plan (ISP)

Assists residents with transferring, lifting, and moving personal items

Assesses environment for residents using adaptive equipment

Implements and follows Individual Support Plan (ISP)

Administration

Communicates with health care professionals, guardians, and program staff to ensure all program documentation requirements are completed according to policy

Communicates results of medical/therapy appointments using Therapy documentation system

Insures proper documentation maintained on THERAP electronic documentation system and client files.

Tracks doctor appointments and insures updates occur as prescribed for each individual served.

Provides job related training to staff as directed

Assists in providing and scheduling appropriate staffing coverage as requested

Assists with maintaining accurate and complete client files, progress reports, and medication reports required by policy or monitoring.

Assists with site compliance with DES, DDD Residential monitoring and DHS licensing regulations, as well as all Corporate, Agency, and Program policies and procedures

Ensures required documentation of client funds according to regulatory requirements, and Corporate, Agency, and Program policies and procedures

Responsible for maintaining appropriate levels of food and supplies, inventories of residents' homes and orders supplies

Attends all required resident, staff, and in-service training and/or meetings

Residential Site Operation

Ensures site equipment is properly maintained

Coordinates monthly fire drill evacuations and First Aid Kit review documentation

Maintains living environment in a clean, sanitary, and safe condition, and completes home safety checklist, as directed

MINIMUM QUALIFICATIONS

Suitable work experience may be considered as transferable skills in order to meet minimum requirements of the position and will be considered by the Corporate Director of Human Resources.

High School

2 years of experience in an independent living setting or residential or day program working with persons with disabilities in a Lead position

Proven ability to lift 60obs 20% of the time

Minimum of 18 years of age as required by licensing

Obtain State of Arizona Level One Fingerprint Clearance Card and pass drug screening

Valid Arizona Driver License, proof of insurance and Arizona Motor Vehicle Division 39-month Motor Vehicle Report

 

DESIRED QUALIFICATIONS

Post-secondary education

Additional years of direct related

Certified Nursing Assistant or other related health care licensing

American Sign Language skills
