Position Purpose

Coordinates and provides casework and supportive services for families in housing- related programs. This position is also responsible for outreach, screening, and enrollment in the SNAP program.  Position may from time to time be stressful and require a high demand of performance. May perform other duties as assigned

Essential Functions

  • Processes self-referrals and referrals from community agencies by providing initial screenings, intakes, interviews, and eligibility assessment of participants
  • Develops individual service plans, in partnership with clients; monitors progress; and coordinates advocacy and follow-up services
  • Conducts group educational classes related to self-sufficiency topics for clients and/or their family members
  • Provides linkages for clients to social service, legal and advocacy services, or other types of community support
  • Participates in the collection and review of all client's confidential documentation as required by the program and/or contract
  • Networks with local agencies and participates in outreach events
  • Processes SNAP applications and benefit screening, using online software
  • Assists with distribution of donated food, clothing, and household goods to families/individuals
  • Maintains individual case files for clients that include plans and strategies for achievement of housing goals and progress
  • Some evening and weekend hours may be required
  • May drive agency or personal vehicle on company business 

Minimum Qualifications 

Suitable work experience may be considered as transferable skills in order to meet minimum requirements of the position and will be considered by the Corporate Director of Human Resources.

  • Associates in social services or equivalent certification/training in related field
  • Ability to relate to individuals/families of varied lifestyles, background, and ethnicities
  • 2 years' experience in providing case management to low-income individuals
  • Bilingual English/ Spanish, verbal and written
  • Skilled in using Microsoft Office Suite
  • Excellent organizational skills
  • Strong communication skills-oral and written communications are clear, concise, and organized using appropriate style, grammar, and tone
  • Ability to stoop, bend, squat, reach and ambulate over uneven floor surfaces
  • Ability to lift 25 lbs.
  • Obtain State of Arizona Level One Fingerprint Clearance Card
  • Valid driver's license for State of Arizona, proof of insurance and minimum standard 39-month MVR report
  • TB skin test and drug screening

Desired Qualifications 

  • Associates in social service or related field
  • Additional experience working with youth in an after-school program
  • Bilingual, English / Spanish verbal
  • Experience with programs serving high-risk youth
