The Operations/Quality Improvement Manager provides information related to quality care of members, property/lease tracking facility maintenance/repair and DHS licensing, supply use under various contracts vehicle maintenance/tracking to meet agency and ADOT standards. This is a billable position, requiring specific site billable units each week. May perform other duties as assigned.

Operations/Quality Improvement Manager Essential Functions

Quality Improvement:

  • Implements site reviews related to facility and safety across CLP
  • Reviews quality of care of consumers through a review of incident reporting, therap notes and site vists tracking safety protocols followed, menu planning, food for three days and other DDD requirements
  • Provides training once a month to Site coordinators on health and safety topics
  • Develops risk management plan related to safety concerns that identifies areas of concern and ways to implement actions to reduce risk related to CLP
  • Address areas to include inspections, emergency procedures, access to aid, protocol for seeking safety, needs of staff and consumers related to safety procedures, and other areas as determined
  • Assists with the care, supervision, support and daily living skills training for individuals with multiple disabilities in a residential setting
  • Supports CLP related to facility and contractual tracking needs
  • Tracks all incident reports and submits a report monthly on issues from the incident reports that will trend programmatic needs of staff training, site concerns and other incident reporting trends
  • Supports and assuring Behavioral plans are developed and implemented.
  • Assist in internal fact findings and investigations with completion of reports to DDD and APS and other entities required
  • May be assigned to various work sites based upon need of CLP
  • Documents implementation of recommended programs, outcomes, progress and concerns regarding facility maintenance, vehicle maintenance and follow up to insure completion of repairs
  • Attends regular meetings i.e. agency, individual client and staff interactions, and in-service training sessions as required by supervisor
  • Works with the Program Managers and communicates regarding individuals, case managers, medical personnel, parents/guardians, day program personnel, neighbors, in a professional manner and appropriate to the needs of the CLP
  • Assists in supporting the program needs to include members, staff and community at large
  • Provides input and feedback to co-workers, supervisors and administration on facility and vehicle needs, improvements and/or ideas for change in the best interest of the individuals served
  • Assist with Incident Reports and Internal Fact Findinngs as directed, and provides assistance with DDD Care Concern requests
  • May drive agency or personal vehicle on company business


  • Oversees CLP facilities and all DRS vehicle tracking
  • Assists with training DRS staff in vehicle, and CLP/COPD facility and safety related topics
  • Sets up and maintains systems for product ordering, maintenance and repair requests and completions, site inspections and vehicle inspections
  • Manages all program site leases; receives lease renewals, processes with CLP and CCS administration, and maintains a current lease file
  • Conducts quarterly walk-throughs of all CLP Program sites with landlord/owner/site supervisor; documents results of walk-throughs, to include assignments for repairs and maintenance
  • Maintains a maintenance repair log for all program sites. Follows up to insure adequate completion of repairs
  • Maintains and distributes owner/landlord contact information
  • Initiates repair and maintenance contact/requests with landlords/owners and completes follow-up regarding completion/issues; documents repair and maintenance results
  • Contacts and maintains appropriate DHS licensing for all sites as required
  • Tracks DHS licenses for renewal and completes licensing applications
  • Meets with DHS Licensing Inspectors to complete review of facilities
  • Assures compliance with site inspections for safety, other
  • Insures documented corrective action and completion of requirements per DHS Licensing inspection results or other site-specific requirements
  • Completes process for appropriate licensing of new CLP group home sites
  • Insures all CLP living environment in a clean, sanitary, neat and safe condition at all times
  • Monitors and assists residents with energy conservation and home safety
  • Assists with the grant preparation for procurement of vehicles as requested
  • Maintains up to date practices, policies, forms and tracking information on all agency vehicles to meet the ADOT contract requirements


Suitable work experience may be considered as transferable skills in order to meet minimum requirements of the position and will be considered by the Corporate Director of Human Resources.

  • Minimum of 3 years of experience in working in the field of Developmenatl Disabilities
  • Excellent verbal and written communication skills
  • Knowledge of the DES/DDD Residential Program Monitoring requirements
  • Proven ability to lift 60 lbs. 15% of the time
  • Obtain State of Arizona Level One Fingerprint Clearance Card and pass drug screening
  • Valid Arizona Driver License, proof of insurance and Arizona Motor Vehicle Division 39-month Motor Vehicle Report


  • Additional years of directly related experience
  • American Sign Language skills
  • Completion of required CLP CCS modules
  • Minimum of 3 years of experience in working in the field of Developmenatl Disabilities
  • Excellent verbal and written communication skills
  • Knowledge of the DES/DDD Residential Program Monitoring requirements
  • Proven ability to lift 60 lbs. 15% of the time
