Program Description : In Home Services/Family Connections provides change-focused interventions to Department of Child Safety (DCS) referred families to achieve Family Connections core outcomes of : Social support, family function, family resources, child well-being, parenting attitutes and behavior and management of parental stress. The Family Connections program works with the family in their homes and assists them to strengthen parental protective factors to reduce the risk of child abuse or neglect and enhance parent/caregiver capacities so that children are safe in their own homes. Case types may include family preservation, reunification or stabilization

The CM II CASE MANAGER LEAD may provide parenting instruction and needed services; provides or arranges for transportation of the client, addresses risks of child abuse and/or neglect in client families. Working in communities and travel to family homes is required. Position may from time to time be stressful and require a high demand of performance. May perform other duties as assigned.

  • Meet with the referred family one to two times per week in their home or community during the length of the service.
  • Utilize strength-based engagement techniques to encourage participation in services and towards goal achievement.
  • Maintain communication with the assigned DCS Specialist regarding family progress or concerns through email and weekly progress reports.
  • Conduct a comprehensive family assessment utilizing established assessment tools, observation and in person interviews.
  • Facilitates or participates with other stakeholders and the family in meetings to review progress, needs, concerns and barriers on a regular basis per contract timelines.
  • With other team members and family, develops a service plan based on the family's needs, assessment and the risks and safety factors identified by the team and DCS.
  • Assists the family to obtain concrete services, with navigating behavioral health, and other services to support the established goals.
  • Is available to provide services on weekends and evenings when necessary; is accessible to families for emergencies.
  • Thoroughly documents all case activity timely and accurately.
  • Submits weekly reports, comprehensive assessment, and service plan timely and accurately.
  • Actively participates as a member of the team.
  • Participates in supervisory meetings with the supervisor, both on an individual basis and as a group; reviews all new cases with a supervisor.
  • Participates in CFT's. TDM's, Foster Care Review Boards, and other meetings as necessary
  • Attends meetings as assigned and appropriate.
  • Participates in training and regular process improvement activities.
  • Completes required initial training for Family Connections and (15) hours of competency-based training per year, to include the Arizona mandated reporter law, cultural competency, how to access Title XIX services, and evidence-based or informed practices.
  • May drive company care or personal vehicle on agency business


Suitable work experience may be considered as transferable skills in order to meet minimum requirements of the position and will be considered by the Corporate Director of Human Resources.

  • Must have a Bachelor of Social Work degree or a related bachelor's degree, with a minimum of one (1) year of human services work related experience.
  • Must be able to work as part of a diverse group.
  • Must have problem-solving ability.
  • Ability to relate well to families of varied lifestyles, backgrounds, and ethnicities.
  • Experience with multi-problem families
  • Has or obtains knowledge and understanding of Social Work Theory, Family-Centered Practice, family engagement, motivational interviewing, and treatment philosophies of family-based services.
  • Valid Arizona Driver License, proof of insurance, Arizona Motor Vehicle Division 39-month Motor Vehicle Report, and reliable, safe transportation


  • Master's degree Social Work, Counseling, or related Human Service field
  • Additional years of directly related experience in a child welfare setting.
  • Knowledge and understanding of Social Work Theory, Family-Centered Practice, family engagement, motivational interviewing, and treatment philosophies of family-based services.
  • Bilingual, English / Spanish verbal and written.
