Position: Lead - Direct Support Professional

Location: Tucson

We are looking for a Lead Direct Care Professional. This person provides oversight, scheduling, direction, monitoring, and coordination of program activities of assigned community-based residences for individuals with developmental disabilities. Assists with the development and review of ISPs, behavior plans, and medication plans. Monitors and maintains client case records ensuring necessary guidelines and standards are followed. Monitors direct care staff, ensures site and equipment are properly maintained. Assists with the development of staff schedules and ensures required documentation is properly completed and filed. Responsible for assuring staff and client coverage to meet contractual obligations, quality care for residents, compliance with all CCS and CLP policies, and assuring licensing standards are met. May drive agency or personal vehicle on company business. Position may from time to time be stressful and require a high demand of performance. May perform other duties as assigned.

Location: Tucson

Three Openings, Full-time, 36 hours weekly

MINIMUM QUALIFICATIONSSuitable work experience may be considered as transferable skills in order to meet minimum requirements of the position and will be considered by the Corporate Director of Human Resources.*Associates or 2 years post-secondary education in social service or related discipline (4 years of directly related experience may substitute for education requirements)
-2 years of supervisory experience
-Excellent verbal and written communication skills
-Knowledge of the DES/DDD Residential Program Monitoring rules
-Experience with writing and implementing objectives and skill plans per ISP guidelines
-Proven ability to lift 60 lbs 15% of the time
-Obtain State of Arizona Level One Fingerprint Clearance Card and pass drug screening
-Valid Arizona Driver License, proof of insurance and Arizona Motor Vehicle Division 39 month Motor Vehicle Report
