The Site Coordinator facilitates the implementation of Individual Service Plans (ISP) and participate in a range of administrative duties including staff, training, scheduling, and oversight. Coach and work within a team of Caregivers supporting individuals we serve in the program. Provide support to individuals receiving services including: assistance with daily activities such as meal preparation and planning; medication administration; and personal care. Duties are split between providing direct support, professional or program activities, and administrative duties. Facilitate community involvement by accompanying individuals on outings in the community or providing transportation to work or other activities.

MINIMUM QUALIFICATIONS FOR THE PROGRAM COORDINATOR

Suitable work experience may be considered as transferable skills in order to meet minimum requirements of the position and will be considered by the Corporate Director of Human Resources.

  • High School diploma or equivalent.
  • One year related work experience.
  • Must be 18 years or older.
  • Excellent verbal and written communication skills with an ability to establish rapport with team members and those we serve.
  • Strong organizational abilities to ensure staffing and schedules are maintained.
  • Knowledge of the DES/DDD Residential Program Monitoring rules
  • Experience with writing and implementing objectives and skill plans per ISP guidelines
  • Proven ability to lift 60 lbs 15% of the time
  • Other licensure or certification if required by regulatory authority
  • Obtain State of Arizona Level One Fingerprint Clearance Card and pass drug screening
  • Valid Arizona Driver License, proof of insurance and Arizona Motor Vehicle Division 39-month Motor Vehicle Report

DESIRED QUALIFICATIONS FOR THE PROGRAM COORDINATOR

  • Supervisory experience
  • Additional years of directly related experience
  • American Sign Language skills
  • Completion of required CLP CDS modules
